Has anyone else killed bosses and not gotten the pattern?

I ran Mythic Skyreach with 2 other JCs, and all 3 of us got the pattern there. But when we killed Iskar, only one person got the pattern. Are these not supposed to be 100% droprates? I assumed that it was personal loot and given to anyone applicable for them.
