Helm visible: Yes please, hiding those octopus-like chin tentacles is a must!
Your pose: Surprise me generally something that looks like I'm in combat? The Warrior Challenge mode shoulders set on fire when you use an ability, I think that would look rather splendid.
General colour scheme: Quite dark, maybe a bronze/red/black background? Something that goes well with the Challenge Mode set / fire.
Any extras: To go with the fiery shoulder effect, I decided to transmog my sword to Blade of Wizardry. If it fits, perhaps this could go on there too?

Edit: An avatar/crop of the face would be greatly appreciated too
