I don't think Hitler was a lunatic, he was perfectly capable of spelling out his ideas and why he believed in them, and he was hardly the only person in Germany to share those views, or else he wouldn't have been able to amass so many followers. To imply that he was only this way because of some mental illness would actually be to absolve him of responsibility.

As far as Neo-Nazis go, they are different from other radical groups in that they largely serve the interest of the powers that be, which is why you have billionaires funding these right wing websites and politicians refusing to denounce them publicly. After all, oppressing minorities can get expensive, much better to let the people handle that for you by filling their minds with propaganda.

Another more classical aspect is upbringing. Parents were Neo-Nazi's and invariably they are. But I think the demographic the Left dislikes the most will always be shifting rightward anyway and its hard to tell what you mean by Nazi since again, definitions of who is and isn't a Nazi really vary.
