If you look more closely at the Y'shaarj incident you'd see that the Titans were panicking when that happened. They sent their legions of Titanforged to create numerous facilities to help Azeroth heal, monitor her health, and promote her healthy growth. Most of those facilities have been destroyed or abandoned, and the Titanforged have for the most part been killed or corrupted and aren't doing their jobs anymore.
TLR it's the difference between being stabbed in a hospital surrounded by the best medical professionals and equipment in the world and being stabbed in the middle of nowhere with no way to get to a hospital and no one nearby has any medical training.
The titan watchers "stabilized" the Y'shaarj wound in order for it to become the well of eternity, it didn't just happen on its own.
Depth, probably. Also, it might be the cumulative damage of ripping out Y'shaarj, the two Sunderings of the World, and finally being stabbed by a sword exuding corrupted energies (which we took care of, but there was still plenty of time for shit to get bad) that have led to the current situation being so dire. In the latter, it wouldn't specifically be due to the stab in and of itself, rather, the stab is the straw that broke the proverbial camel's back and breached a threshold.

Well that sword had some sort of corruptive energy that the Artifact Weapons absorbed, and the only thing attempting to treat it are some mortals from Earthen Ring / Cenarion Circle and the Speaker.
Sometimes I think that if M.O.T.H.E.R. (or any other Titanic Watcher that cares like her) acted sooner, the crysis would've been smaller. But we've waited too long, and if I'm not mistaken, it was announced that in the next patch some things and beings fueled by this "Titan'ish arcane power" will be losing their power too. But still: the heck are those Titanforged in Ulduar doing? Odyn is free already, returned to Ulduar, and... his Valarjar collected some Azerite. Nice.
I wonder if Sargeras' attack had some sort of hidden effect. Maybe he managed to implant a portion of his soul onto Azeroth, like he did before with with Aegwynn.

Azshara reveals pretty much what the "King of Diamonds has been made a pawn" line means, based on her dialogue around the Heart of Azeroth. She intends to use the super-powered Heart of Azeroth as a means of unlocking N'Zoth's prison - and the recent empowerment of it, along with the amount of Azerite it has soaked up, has made it the ideal tool. She is more or less arrogantly confirming this was always in the cards, and says that it is fitting that Magni has been made a pawn in all this by giving the Champion the means to finally free the Old God.

Possibly Sargeras infused Gorribal with Argus' essence (Argus was/is the Titan of Death) to kill Azeroth if he failed to corrupt her.
Gorribal poisonned Azeroth, so instead of a similar wound than the one left by Y'Shaarj, Argus' essence acted like the Ebola virus.
So now Azeroth is bleeding from everywhere through her skin (earth), her blood is tainted by the essence of the Titan of Death.
This might be why Azerite is gold (Azeroth's blood true color) with blue (Argus's blood/essence).
This should explain why people in contact with Azerite (like Sylvanas) engaged in war after having visions of death, and the more they have to more extreme they go.
This might explain why the nascent Titan is not healing despite all the Azerite we collect with the Heart of Azeroth and why we need to empower it in 8.2
Also, the corruption of Azeroth's blood might be the reason why N'Zoth is trying to team up with us, making things happen to escape from its prison and probably make a bargain with us, pretending he's the one that could prevent the death of Azeroth.
Are we sure Azeroth is actually dying? At this point I'm assuming it's just NZoth putting on a girly voice and talking to Magni.

Let's see:
1. There are no titans to watch over Azeroth.
2. There are no facilities to heal/monitor her.
3. Sargeras' magics, which are far beyond that of even the Titan Pantheon's. And yes, the magics are still inside of her. We only stopped the magics from continuing to poison her.
4. The Sword's plunged right by, if not, onto Azeroth's heart.
And 5. Sargeras' blade is basically the Planet's size, and the wound was heavily nerfed in size, when shown in game.
So, yeah.

Oh, and ripping a Tick off of you, and getting stabbed in the chest by a blade are 2 completely different pains.

Sageras ultimate goal was to defeat the Old Gods.
He want to achive that by killing all living species so the Old Gods have no one to command.
Why would Sageras Stab Azeroth just to jump start the escape od the Old God?
