It would be bad. For everyone. I have my doubts either side would be interested in a full scale to the death war. Odds are as bad as it sounds a proxy war would pop up and both sides would duke it out in the middle of some third world hell hole. Philippines seems to be volunteering for the duty a lot lately. That way a majority of the casualties will just be flips and both sides could test each others muscle in the air and sea while "supplying" friendly forces from "oppressors" and then in the end whoever loses can just claim "they did their best for the cause of (freedom - US / asian prosperity - China) and be able to still walk away without entering the nuke codes. But then again we have the orange mad man in the white house. He would likely rather see the world reset than lose face on TV.

US would annex Canada, the UN would disband, the US will experiment with psychotic drugs and armor which can make a man as strong as a bear with twice the brain.
China would invade Alaska and develop technology which can turn a person invisible.

There would be no clear benefit to either side, so we'd get halfway through the war and everybody would stop and go "Wait, why are we doing this again?" then team up and invade the real threat: Uruguay.

Should such a war happen, it would start, almost timidly, using conventional forces and weapons, including cyber warfare. Russia and Europe would show the common sense to stay the fuck out of it unless they had no other choice. The Pacific theater would get really heated, with water and air battles sparking all over the place. China's weapon quality is, from what I understand, not as good as the US's, but they easily have the quantity to back it up.

Both sides would avoid the "point of no return" of a ground invasion as long as strategically possible, hoping the other would give up and admit defeat. But once that happens, the war would pretty much instantly go nuclear, destroying both countries in all meaningful ways plus anyone else who jumped in as well -- which could very well involve Russia (see also: Aug 6-9 1945) and probably NATO as well. The resulting firestorm would basically end life on this planet as we know it.
