I wonder if the had some junior local counsel do it, and he thought "Oh, no-one will even see the app for hours and hours, so it won't be accessible!". I mean, I worked for a major city law firm in London, top 30, and whilst did little IP work, we certainly had people make mistakes of the nature on occasion (mostly trainees but associates and at least one partner).
