My buddy has a milk allergy and would probably not like getting cheese on a hamburger.
If I want a cheeseburger...I'll order a cheeseburger? The cheese is just an extra, same as the pickles....ketchup or whatever crap you wanna put on it. A burger should be just a burger and whatever else you want on it you need to tell them to add it. I hated having to pull those disgusting ass pickles off because no matter how many times you tell McDonalds no pickles...they never fucking listen.

Some people are allergic to dairy, so they will need it without cheese.
If they have cheese on by default, they need to make it extremely clear that you need to ask for no cheese specifically, and need to be carefully considered as any error will land them a massive lawsuit.
That is why people rather be on the safe side and have no cheese by default.
Burgers are not very healthy and I barely eat them. Cheese makes them more unhealthy.

As a side to that, the OP pic is a cheeseburger. If you're ordering a product called a double cheeseburger, you should be expecting it to come with cheese. If you're ordering something that naturally comes with cheese, yeah, cheese will be on the burger
It HAS to be optional.
A cheese-loving customer had a burger without cheese: one angry customer, might need a free cheese burger to make up for it.
A diary-allergic person had a cheese burger: potentially a dead customer, and a massive lawsuit to come.
As a safety measure it is in company's best interest to have no cheese by default, that way it is an added layer of control in the case of duty of care.
I thought most fast food places did that already. I know at least the local McDonald's here has a cheeseburger or a hamburger option, and you ask for either specifically.
Although that's just for the very basic one. I think the bigger stuff (quarter pounder, etc) just assumes you get cheese, and you'd have to specifically ask if you didn't want it.

Show me one place in the world where McD's isn't the most convenient option for a burger. Believe me when I tell you that even Bumfuck, Norway get's those damn vinegar-drowned cucumbers on our fucking burgers. Even after politely, in the simplest possible sentence I can construct, I ask for no pickles.
