It definitely is the injury from Saurfang. Look at her right eye. It literally is leaking energy and you can see a scar also leaking that same energy.

I made the same comment on WoWHead... Sylvanas eyes are the actuall! Not only the injured eye but you can see even the smeared mascara...

WoWHead is stupid, because it's Dark Ranger and not Sylvanas. Chinese site just incorrectly placed her there, as before she was placed to Night Elves art.

I'm not seeing the injured eye no matter how far I zoom in, and I'm pretty sure the smeared mascara doesn't mean anything either. But this leak of the Collector's Edition rewards mentioned a "Scarred Sylvanas" spray for Overwatch.

Her unit always had blood under her eyes though.

But what happened was in vanilla they used a Night Elf model for her and somehow the NE tattoo markings became a part of her appearance. BfA finally corrected that back to her original design.

Food for thought: if Shadowlands are an "Outlands" version of Northrend in some way, then they'll use Ner'zhul as a lore reason because he was responsible for both.

And that's why you should always get some kind of insurance. If dropping your helm by accident has a chance to open a rift to the Shadowlands, well... it's a little bit sketchy to say the least.

I don't recall her eye getting injured that badly at any point in the story. Didn't she just get a tiny scratch on her cheek?

EDIT: Never mind, it did get injured. It's still a very tiny scratch.
